Do you have your documents ready?
Proof of 30-day gross income for every household member ages 18 and older (Wages, Social Security Benefits, TANF, AABD, Unemployment, proof of cash wages, etc.)
Income for children who receive child support or Social Security Benefits
Social Security cards for every household member. Proof of Social Security numbers of all household members. For proof of Social Security or Supplemental Security income benefits go to socialsecurity.gov/myaccount for an instant letter.
Current heat and electric bills if you pay for your home energy directly. (Must provide entire bill)
Applicants that have their utilities included in the rent must bring proof of rental agreement stating monthly rental amount, that utilities are included, and landlord contact information. (Rent costs must be greater than 30% of current household 30-day gross income)
Additional Documents to Apply for the Furnace Repair & Replacement Program
Two proofs of home ownership, one of which must be the current property tax bill that shows the customer’s full name and property address. The second proof can be a mortgage or loan statement dated within the last 90 days or any Recorded County Court Record. If mobile-homeowner only the vehicle title is needed.
Application Process

Fill out the pre-application online to see if you are eligible.
If you do not have access to a computer, please reach out to our intake sites via phone to get the ball rolling. Locate your local site here.
- If you provided an email address, you will receive a confirmation email with instructions on how to complete your application by uploading your documents within 72 hours.
- If you did not provide an email address, you will receive a confirmation letter in the mail with instructions on how to complete your application by uploading your documents within 2-3 weeks.

Send Your Documents
Your application is not complete until we receive all necessary documentation. Please note that your application ID will be included in your confirmation email or letter. You can send your documents online, via fax, or by mail.
SUBMITTING DOCUMENTS ONLINE
If your documents are on your computer, you can upload them as soon as you receive your email confirmation after submitting the pre-application.

In the third tab of the application portal, agree to the terms and conditions and input your eSignature.
